Reconstructive Urology

The oxford dictionary meaning of Reconstruction is “the activity of repairing something that has been damaged, destroyed, disfigured or dysfunctional”. These very principles are applied in Reconstructive Urology. This subspecialty is a combination of art, science and wisdom. Patience, imagination and persistence are the prerequisites of this challenging field.

The reconstruction is required for the damaged urological organs due to trauma, infection or inflammation, congenitally disfigured organs and dysfunctional organs to restore the continuity and function.

Subsections are as follows:

Urethral reconstruction and Hypospadias: This is the largest area of Reconstructive Urology. In fact the measure of any training center is decided by the volume of workload of urethroplasty and hypospadias surgery. Last three decades have shown a great interest in urethral reconstruction world over. The volume of research in this single entity of reconstruction has resulted in huge swing in management. Based on the fundamental principles laid down earlier, several building blocks have been built. The outcome measures show improved results in both the primary and redo cases. In the recent years there is a surge in the interest in female urethral reconstruction.

Genital Reconstruction: This section consists of reconstruction of deformed male genitalia. It includes both congenital such as chordee, scrotal malformation and malpositioning, intersex as well as acquired such as Peyronie’s disease, post traumatic and post inflammatory genital deformity. Individualistic approach is needed in these challenging cases.

Prosthetics in Urology: In this section the clinician is involved in placing well assessed implants such as artificial urinary sphincter, male slings, ATOMS, penile prosthesis and occasionally testicular prosthesis. The cost may be an issue.

Gender affirmation:An enigmatic area for a day today Reconstructionist, this division has started taking roots in our country. Gender dysphoria is increasingly recognized DSM-5 condition. It needs multidisciplinary approach with Reconstructive Urologist having to take up role of gender reassignment surgery.

Neuro-urology and upper tract reconstruction: The reconstructive aspects of this area includes functional redesigning of urinary tract. It incorporates cystoplasty for high pressure or small capacity bladders, neo-bladders, ileal ureters. There is significant overlap with other disciplines of Urology.

In order to achieve our goals we plan to develop many sections of this subspecialty. We will identify and create centers of excellence. We will cultivate eagerness to publish and present. We will generate a comprehensive database and design collaborative studies amongst the centers across the nation. We plan to interact with international reconstructive groups. Overall we will create a subspecialty with robust team to support individuals and programs.

Our team envisages Reconstructive Urology subspecialty to become the choice of many if not all young urologist of India. As we encompass various training modules, we strive to expand so that the art and science of this field makes substantial change in the quality of life of the sufferers of the unfortunate ailments.
